- Cook penne in salted boiling water until al dente. Reserve 1/2 cup pasta water, then drain.
- In a large skillet, heat olive oil over medium heat. Add onion and cook 3–4 minutes until softened. Stir in garlic and red pepper flakes; cook 30 seconds.
- Add crushed tomatoes, vodka, salt, and black pepper. Simmer 8–10 minutes to let flavors blend and alcohol cook off.
- Stir in heavy cream and Parmesan cheese. Simmer another 2 minutes until sauce thickens slightly.
- Add cooked penne and toss until coated, adding reserved pasta water as needed.
- Garnish with fresh basil and serve hot with extra Parmesan.
